Check-In For Strikerfest

ATTENTION ALL COACHES/TEAM MANAGERS

 

Team check-in for the Adidas StrikerFest 2011 Tournament will be on Tuesday, July 26, 2011 at Scoreboard Sports, 971 South State Street in Orem from 5:00 pm to 9:00 pm. 

 

Each team will be required to bring the following to Team Check-in:

1.  An official roster generated by Affinity (some teams have a Got Soccer roster or one from their own registration system if they are coming from out of state)

2.  6 "Travel Rosters" generated by the Affinity System or current UYSA Player cards for each person -- (We will have to determine if we are going to accept player cards or only the rosters and how many they need to bring.  They will need one for each game they have a possibility of playing).

3.  Specialty Player Action Form for each guest player along with a valid player card or have them listed on the "Travel Roster"

4.  UYSA Registration Form with parents' signatures in two places or a Medical Release Form available at www.,utahyouthsoccer.net for each player.

Rosters will be finalized at Team Check-in.  No players will be allowed to be added to the roster after Team Check-in.  So if you have a player coming late to the tournament, make sure they are on the roster to begin with.

You will be allowed to have a maximum of 5 guest players on your roster.  You must have the following paperwork for each guest player.

1.        A copy of the player’s registration/medical waiver form.

2.       A signed Participant Agreement, Acknowledgment of Risk Form (available at www.utahfc.org)

3.       A laminated, valid player pass.

4.       A completed Specialty Player action form.  (available at www.utahyouthsoccer.net)  Players or teams not presenting the required documents at check-in will not be allowed to play.  No exceptions can or will be made.   No guest players may be added to your roster after team check-in.
